YM360 Waiver:
Please print and fill out, THIS DOCUMENT MUST BE NOTARIZED before Parent Meeting. CLICK HERE if you have any questions regarding this document.
Special Needs Request Form:
This form is for any special needs of your student, including Gluten Free meal needs. This item only needs printed, filled out and turned in at the parent meeting.
Optional Medical Information Form:
This form is optional, if you would like to fill this out with your students medications and instructions. This form is NOT REQUIRED.
EMERGE Release of Liability:
This form is the typical release waiver but for the church, no need to notarize this one.
Glorieta Adventure Camp Registration:

Side Note
This registration with ask you to purchase camp bucks, PLEASE DO NOT. These are historically difficult to manage and I don’t want any student left without money. Given the camp does not use cash, please send money with your student via a prepaid Visa Gift Card. This will give freedom of use at camp and any stops we make going to or from camp. Leaders/Over 18:
Students over 18 years old unfortunately need to have a background check, please CLICK HERE and fill out the form and we will take care of the rest!
Adult Leaders please CLICK HERE for the background check form, and each adult MUST FINISH the videos below to be a part of camp.
